Population Care Coordinator Program

Managed Care Organizations are increasingly faced with growing expectations—from healthcare providers, shareholders, and the public—to improve patient care outcomes while simultaneously lowering costs. Duke’s Population Care Coordinator Program (PCCP) uses innovative and flexible learning methods to help you achieve bottom-line results.

  • Customized: Curriculum built to fit your organization's needs
  • Cost-effective: Better health care outcomes for patients at a lower cost
  • Rapid deployment: 14 weeks from student intake to certificate completion
  • Participant incentives: Nurse participants have the option to earn 9 graduate credits and 90 contact hours for continuing education from the Duke University School of Nursing
  • Convenience: Duke brings the program to you (optional)

The collaborative training program is delivered in an online format, supplemented with three intensive face-to-face sessions. A residency component serves as the capstone experience, integrating coursework with skills learned, to give students real-world understanding and experience in the role of Population Care Coordinator.
